A 57 year old female presents with a 5 week history of a new mole on the left side of the back. The clinical appearances are of a 6 mm firm papule with central bluish pigment. The patient has a previous medical history of an ocular melanoma 7 years ago.

An 11-year-old boy presented with a symmetrical light brown lesion on the left forearm, measuring 5 mm in greatest diameter. The lesion slowly increased in size in the recent months. A conservative excision was performed.

A 21-year-old pregnant woman presented with an unevenly pigmented lesion on the vulva, measuring 15 mm in greatest diameter. The lesion was asymmetrical and slightly raised on clinical examination. A diagnosis of melanoma was suggested on clinical grounds and conservative excision was performed as an initial procedure.

42 year-old female.
Congenital lesion on the occipital region: large plaque with 20x15 cm, with dark colour varying from brown to blue, with transverse and longitudinal sulci on the surface, conferring the lesion a cerebriform appearance. The lesion was asymptomatic.
In the previous year there was a rapid increase in its volume, with an ulcerated, friable non-pigmented tumour appearing over the surface of the pre-existing lesion.

A 15-year-old girl (July 1993) first presented with a 3-year history of a slowly growing nodule that had occurred in a large flat congenital nevus, which covered almost half of the scalp in the frontotemporoparietooccipital area. The clinical diagnosis was an adnexal tumor or fibrolipoma. The lesion was surgically removed. One (June 1994) and five years (October 1998) later, two 0.8 cm white to grey nodular lesions occurred in the same site. The lesions were excised. Seven years after the first surgery (December 2000), metastases in axillary lymph nodes and the right breast were found, and few months later (March 2001) distant metastases involving the spine and liver were detected. Nine months later (December 2001) the patient died at the age of 23 years.

A 63-year-old man with a 2-year history of a primary desmoplastic malignant melanoma on the right heel with lymph node metastasis in the right groin, presented with a 7 x 6.5 x 3.5-cm painless mass in the right inguinal area. The mass was surgically removed. Seven months later the patient died with multiple liver and bone metastases.
